Require human approval for anything destructive, irreversible, outbound or that spends money
Log inputs, decision, output, duration, cost and a searchable run id for every single run
Put an error branch and a heartbeat on every production flow so silence means healthy, not dead
Use idempotency keys to stop retries double-sending, and measure correct resolution instead of green runs
